Requirements for your application package:
  1. ProMed Application completed and signed. Click Here to Download and Submit.
  2. U.S. Form I-9 completed and signed. (government employment eligibility form) Click Here to Download - then complete and attach to your email to us
  3. Copies of 2 forms of ID (Examples are: State issued Drivers License and a Government issued Social Security Card OR a Government issued Passport. More information regarding acceptable forms of identification can be found in the instructions portion of the 1-9 PDF (See Item 2 to Download.)
  4. Copy of current drug screen ( test results must be no more than 6 months old from current date to be accepted)-If you do not have a current drug screen, please contact the main office so that an appointment can be made for you.
  5. Authenticated Background Check - If you have not had a background check in the last 6 months, Promed will perform one using any and all information applicant provides
  6. Current CPR certification (issued by American Heart)
  7. Basic Life Support (issued by American Heart) (RN and LPN Only)
  8. PPD (TB test must be no older than 1 year)
  9. Current Immunization Records - Including H1N1 (if you have received it)
  10. Current Physical (must be no older than 1 year from current date)
  11. Copy of any Certifications you currently hold (Required for CNAs, Optional for LPN, RN and Respiratory Therapists)
  12. Copy of current license (RN, LPN and Respiratory Therapists Only)
